The Manchester Health Department’s Public Health and Safety Team’s Community Health Workers (CHWs) address public safety and health issues within the City’s 12 Wards through a partnership with the Manchester Police Department. Care coordination services by the CHW team include but are not limited to: access to basic resources, such as food, navigating health care and other support services, assistance with basic housing and safety concerns, and help with enrolling in programs/services. The team is
multicultural and collectively speaks 9 languages in addition to English (Spanish, French, Nepali, Hindi, Swahili, Kinyarwanda, Kirundi, Lingala, and Yoruba).

Support for Students Experiencing Homelessness
The City of Manchester Health Department (MHD) has partnered with the Manchester School District to provide an experienced Community Health Worker to assist students/families experiencing homelessness with basic needs. The CHW can connect students/families to medical, dental, and vision providers; help them secure health insurance; assist them in completing applications or determining eligibility for Medicaid, WIC/Food Stamps, fuel/rental assistance, and other financial assistance; and connect
them to resources for food, clothing, housing, and other basic needs and social supports.
Asthma Education and Resources
Asthma is a chronic disease that often requires daily attention. The Manchester Health Department Asthma Education and Outreach Program provides families with education and outreach services in the home setting for children age 1-18 years who have been diagnosed with asthma. Our Asthma Educators assist families in understanding how to manage their child’s asthma, their child’s asthma medications, and how the environment can effect asthma. Equine Assisted Services
Through our Unmounted Equine Programs, we also support youth, families and military veterans who have been through tragic loss, high-conflict divorce, physical or mental abuse, suicide, PTSD, substance abuse and bullying.

Child and Adolescent Department at MHCGM
The Child and Adolescent Department at MHCGM offers family, individual and group therapy services. Additionally we provide case management and medication services. Our services are offered in the community, schools, home and office.
Bookmobile
The Manchester School District, in partnership with the Manchester City Library, will run a Bookmobile, made possible through generous donations from local businesses, community organizations and volunteers.
The Bookmobile is staffed by Manchester School District educators city librarians and volunteers. At each stop, Manchester students can choose a FREE BOOK to take home, enjoy a snack and activity. All Manchester children of any age are welcome to attend.
